Complete and submit the form below. If the new school you refer meets the gross sales minimum requirement, your school will receive a check for $500!

Rules:

• QSP Partner School has run a QSP Reader's Digest fundraising program within the previous 12 months.
• No group in the new School has run a QSP fundraiser in prior school year.
• This Refer-A-Friend form must be received in order for QSP Partner School to be eligible for the $500 bonus.
• The new school must run a magazine fundraiser that generates at least $3,000 gross sales in order for the QSP Partner School to earn the $500 bonus.
• Partner School can only refer one group in a new school. Partner School cannot refer a group within its own school.

Checks will be sent out to the local sales manager for distribution after sales have been calculated, typically in January and June. QSP Reader’s Digest reserves the right to make changes to the terms and conditions of this program at any time.
